Loose steering

Has any one experienced the forks coming loose, and when you put your front break on the handle bars shake back and forward. this happened to me a few weeks ago but I took the front off and tightened the main bolt. I think this happened as a result of going over a bump at about 40mph. Has anyone else had this problem?
