Sounds to me like a dirty carb problem. I would Check out the corresponding carb to that cylinder. You have eliminated electrical already. Valves should be calb. Every 4k miles but they would ALL be missing not just one set. Dirty Carb(s) are the suspects here seems like from ur info.
