Problem changing gear

Problem changing gear

2003 Innova 125 (10000 miles)- having occasional difficulty changing gears 1st to 2nd. Sometimes a loud, juddering clunk from the engine, sometimes unable to change up, yet at other times no problem and works smoothly. Any advice would be appreciated.
